Waiver and liability release
Parents and guardians must review and accept this waiver before registering a player for a Ballerz United Basketball tryout.
Participation acknowledgement
I understand that basketball tryouts, training, conditioning, games, and related activities involve physical exertion and inherent risks, including falls, collisions, illness, and injury. I confirm that the participant is physically able to take part in tryout activities, except for any limitations I disclose during registration.
Medical authorization
If I cannot be reached in an emergency, I authorize coaches, staff, volunteers, or event personnel to seek emergency medical care for the participant. I understand that I am responsible for costs related to medical treatment, transportation, or follow-up care.
Release of liability
To the fullest extent permitted by law, I release and hold harmless Ballerz United Basketball, its coaches, staff, volunteers, facility partners, and representatives from claims arising out of ordinary negligence connected with the participant's tryout attendance or participation.
Code of conduct
I agree that the participant and accompanying guests will follow coach instructions, facility rules, and respectful conduct expectations. Ballerz United Basketball may remove a participant or guest from tryout activities for unsafe, disruptive, or inappropriate behavior.
Consent
By checking the waiver box on the tryout registration form, I confirm that I am the participant's parent or legal guardian, that the information I provide is accurate, and that I accept this waiver on behalf of myself and the participant.
